Please read this liability waiver and hold harmless agreement carefully before participating in any activities at Airlock Jiu Jitsu LLC. This agreement affects your legal rights. By signing this document, you acknowledge that you understand and accept the terms outlined below.

1. Assumption of Risk: I, the undersigned participant (or parent/legal guardian if participant is a minor), understand that Brazilian Jiu Jitsu (BJJ) training, open mats, and other related activities offered by Airlock Jiu Jitsu LLC (hereinafter referred to as "the School") involve inherent risks of physical injury. These risks may include, but are not limited to, sprains, fractures, bruises, cuts, concussions, and other serious injuries. I voluntarily assume all such risks associated with my (or my child's) participation in BJJ activities at the School.

2. Physical Condition: I hereby certify that I (or my child) am in good physical condition and have no known medical conditions that would prevent me (or my child) from safely participating in BJJ activities. I understand that it is my responsibility to consult with a medical professional prior to participating if I (or my child) have any concerns or doubts about my (or my child's) physical health and ability to engage in such activities.

3. Waiver and Release of Liability: In consideration of being allowed to participate in BJJ activities at the School, I hereby release, waive, discharge, and covenant not to sue Airlock Jiu Jitsu LLC, its owners, instructors, employees, agents, and volunteers from any and all liability, claims, demands, actions, or rights of action, whether for personal injury, property damage, or otherwise, arising out of or related to my (or my child's) participation in BJJ activities, including but not limited to any negligence, breach of contract, or breach of statutory duty by the School or its staff.

4. Hold Harmless Agreement: I agree to indemnify, defend, and hold harmless Airlock Jiu Jitsu LLC, its owners, instructors, employees, agents, and volunteers from any and all liabilities, damages, losses, expenses, claims, or demands, including reasonable attorneys' fees, arising out of or related to my (or my child's) participation in BJJ activities at the School, including but not limited to any injuries or damages caused by my (or my child's) actions or negligence.

5. Insurance: I understand that Airlock Jiu Jitsu LLC does not provide medical coverage for participants. I acknowledge that it is my responsibility to obtain and maintain my (or my child's) own health and medical insurance coverage.

6. Consent to Medical Treatment: In the event of any injury or illness that may occur during my (or my child's) participation in BJJ activities at the School, I authorize the School and its staff to secure and administer medical treatment as deemed necessary. I understand that I will be responsible for any associated medical expenses.

7. I hereby grant Airlock Jiu Jitsu irrevocable permission to publish videos/photos of ME/MY CHILD taken at Airlock Jiu Jitsu for the purpose of promoting their services and products. These videos/images may be published in any manner, including (but not limited to) social media, websites, press releases, etc. Furthermore, I will hold harmless Airlock Jiu Jitsu and their legal representatives/appointees, from any liability by virtue of editing, cropping, color/exposure shifts, etc. that may occur in reproducing videos/photos.
